Here's to manifesting that everyone matches at a place that might not necessarily be their #1, but a place where they'll be happiest and able to achieve their future goals.

We've all given ourselves up to a 2+ year commitment: writing 9-hour exams, investing in electives, dressing to deadlines, coordinating between medical bodies, smiling for interviews, and anticipating life-changing notifications.

And finally, making big decisions on ranking places we've never been to, hoping it'll be the best for us.

It's all come to an end... an end of an era. We'll remember every moment, phase, emotion, and importance.

I remember the countless nights spent studying, the early morning coffee runs, and the overwhelming sense of uncertainty that often felt suffocating.

But we made it. We survived the chaos, and we're stronger because of it.

I truly manifest that each one of us gets matched to a place that welcomes us as our new home, nurtures our hard work, and manifests it into future dream opportunities.

But tbh, we're already living our dream. Being done with ROL? Never thought this day would come so quick.

So appreciate yourselves and how far you've come... don't doubt your ROL, even if it's that last-minute change. Accept the decisions made as ones made from a lot of thought and feeling.

This has been an incredible journey! I have received so much support from so many wonderful people in this platform during my most challenging moments. There were times when it seemed impossible: taking exams for two countries, leaving my family for two years to complete rotations, and even working night shifts stocking shelves at Walmart just to pay the bills.

As an international medical graduate, matching in Canada is even more challenging than in the USA—but my country believed in me. I cherished every minute of my rotations and deeply appreciated the support from my American preceptors and fellow students. Matching at home feels incredibly sweet and rewarding.

Completing two Canadian licensing exams and Steps 1–3 in just nine months was grueling—not to mention the interviews (six in the USA and one in Canada). Withdrawing from the NRMP after matching in Canada made me realize just how special it is to match at home: no J-1 visa and no mandatory two-year return requirement.

My journey has not been without obstacles. I experienced an 11-year gap between my basic sciences and clinical training due to a severe nerve injury from a motor vehicle accident. I underwent extensive therapy and surgery, and three years ago, I returned to complete 88 weeks of rotations in both the US and Canada.

At 43, with two wonderful kids, a loving wife, and an 80-year-old mother to care for—and even with a paralyzed right foot—I never lost sight of my dream to become a family physician serving my community. Today, that dream has been officially accomplished.

Now, it's time to serve and give back. Please, do not give up, no matter the circumstances. Follow your heart and do things for the right reasons, and the universe will be on your side.

Then they had the whole interview to experience the vibes. It’s all personal to each. Just writing help me with my ROL without proper information about what they are looking is so useless and immature approach to things.

  • User:Choose WISELY the places you’re going to signal.

    • User:This. Do not overshoot and waste your signals on competitive programs that are more likely to be signalled by a lot of other applicants. Take a good, long, objective look at your application and based on that, cast a wide enough net with some high tier, some mid tier and some low tier community programs to be on the safe side.

      • User:I also feel the Silver Signalling for IM especially if you’re an IMG is a myth. None of the Silver signals sent invites as far as I know. Next year, we do not know the tier format of signals yet. But if one asks me, i would say we are better off with 7 non tier signals viz a viz 3 gold and 12 silvers. 3 golds - most probably will send you invites. 12 silvers - definitely won’t coz they might have gotten golds too. So you’re ending up with 2 to 3 ivs. But last year 6 to 5 signals worked out of 7. Don’t rely on your signals. Just keep in touch with the people. Attend conferences, build contacts, ask the PDs what they look for.. try getting strong letters.

        • User:HOT TAKE: honestly this is weird i was ignored by three mid gold signals and was given interviews by very reputable silver signals. It's a fucking gamble.

        • User:I received all my interviews from signals. 1 from gold and 3 from signals. And 2 of my silver signals were out of my geo pref and got IVs from them. Also, I got 0 IVs from tristate area.

  • User:The factor that affected my application the most was my YOG. My roster is entirely different from <YOG people with similar backgrounds. I got my way around it by strengthening other parts of my CV so I ended up getting >15 interviews combined both IM and FM with two prematch programs. Mistakes - I should've taken Step 3 already, especially with a YOG problem. Minor things - I made my CV well-rounded. I made my app genuine, and I intend to root myself in the community should a program take a chance on me (which is true). Advice: Try to be as authentic as you can and make it intentional - meaning you say what you mean and you mean what you say. There is no "formula" that will get you sure interviews. But of course make sure all bases are covered (good scores, rotations, research) etc. EDIT: Whoops I didn't expect this to gain attention. To those who sent a message in my inbox, I'll get back to you, I'm just studying for Step 3, I gotta say goodbye to social media for now. Lol

    • User:That is maturity. Twelve years ago, I did the same thing with 5 YOG, and it worked perfectly with 10 IVs. I barely had money for 30 programs, so I simply took what I could and chose the ones with the most opportunities, geographically and all that. It worked before, and it works now. I recently got a lot of negative points on a comment because I told a kid the truth. Always be honest, always be authentic. In the end, they are not fools, and they are tired of seeing everything. You won’t impress them; you just have to be yourself, and if you fit, they will call you. It’s that simple.

    • User:This. Don’t underestimate YOG. I ended up with 15 ranks as well. But I felt I got filtered out from most competitive programs due to YOG.

  • User:Do your research on the specialties that you may possibly consider. I decided on EM late and was completely unprepared for the need for SLOE vs a regular LOR. The only reason I have a chance to Match EM without an SLOE and low Step 2 score is a strong PS and LORs. I see a lot of people being very flippant about their PS. The easiest way to differentiate yourself from other candidates is to tell an engaging narrative that highlights your unique strengths and shows growth over your journey in medicine. You are applying to be a trainee, so demonstrating teachability is crucial. If you're a less competitive candidate, take every opportunity to meet programs in person to make a stronger connection than virtual IVs. Finally, read every report you can from NRMP, and use Residency Explorer to understand how competitive you are. If you can't objectively evaluate yourself, you will waste a lot of time and money chasing a pipe dream.

      • User:6. I'm sure I got screened out a lot! I guess some programs really do a holistic review 🙏 That reminded me of another mistake, take Step 2 prior to applying!

          • User:Thanks! I'm very grateful. Geo prefs: South Atlantic, East South Central, Rural or Suburban All my IVs were suburban outside of my preferred regions.

        • User:when would you say latest to take step 2

          • User:I believe the max time it takes to get your score back is 3 weeks. You can transmit your scores to ERAS the same day, so you should get your report before the first day programs can see your app. If you have to pass comp before Step 2 make sure you know your school's policy. That's what screwed me. I didn't realize I had to sign up a month in advance to take Comp.

  • User:I am not an applicant, but worked with 140 applicants this year and another 120 last year. Here are the biggest things that I think negatively impact matching success for IMGs. 1. Not having a native English speaker/writer review the ERAS application and PS. 2. A personal statement that is a rehashing of the CV rather than about the candidate as a person. 3. Not carefully researching programs and applying to those that align with their profile (e.g., YOG, visa needs, Step scores) 4. Assuming that a crazy high Step 2 score is what materials (in fact, I feel that a crazy high Step 2 score might be seen as a negative because of the cheating scandal) 5. Over-applying to a zillion programs (I know it's out of desperation), but when this happens, it makes it hard for programs to carefully review programs 6. Not having LORs (US LORs) in the specialty being applied to 7. Not practicing for the interviews 8. Not having completed Step 3 by application time (or by the end of November) if at all possible (I realize in many instances, this is not feasible, but having it completed can take that off the plate if you have a significant YOG issue or a low Step 2 score \[below 220\] or a failed Step 1, Step 2, or Step 3 attempt 9. Not having a complete application by the date programs can begin reviewing applications 10. Relying too heavily on people giving advice based only on their experience (an n = 1) or others not well-versed in the ERAS/Residency CAS/SF match process 11. Not connecting the 10 experiences to their key characteristics of the experience (e.g., communication, teamwork, leadership, empathy/compassion, reliability/dependability, etc.) and their own personal characteristics. It's a way to get more about *you* into the ERAS application. 12. Not aligning signals and geographic preferences. Programs are looking at signals for their 1st tier of interviewees. The data is clear on this. 13. Over-reaching when applying to programs. A few stretch programs are fine, but try to be realistic those that you apply for. 14. Not beginning soon enough...March of the cycle year is when you should begin working on ERAS, your PS, getting LORs lined up, researching programs, etc. That way you're not panicking in July/August/September.

  • User:1. **Signal** places you know you would be competitive. This is very important, you don't wanna waste signals on a bunch of reach programs that everybody wants to get into. I personally wanted to signal reach programs, which had matched IMGs in the past, but upon more research, I learned that all of those IMGs had done research fellowships at that institution. It would have been a waste of a signal. In fact, my golds were strategically used towards programs. I knew I would be very competitive at. 1. **Networking, networking, networking!** I got interviews at places that would've otherwise pass me up due to the networking I did during rotations. Those relationships helped me a lot. 1. This one is a bit abstract, but I think figuring out your **personal story** and your **brand** is important. There are many things in my hobbies section and in my personal statement that I think sent me apart from other applicants with similar scores and research. Everybody is unique, find out what's unique about you and highlight as much as you can.

    • User:Yes. Most IMG friendly community programs (that are considered malignant or mid/low-tier by many) have a step2 cutoff as around 255-260. There is no clear ofifical source for this info, but you can definitely see the pattern on spreadsheet interview invites. Suspiciously low minimum score requierement that programs list on their application info page is pretty much bs and I think disrespectful. "We ReQuiRe a MiniMuM sCoRe of 220 on sTeP 2" while their PGY-1 class have an average of 25x or something. You can't find score info easily, unfortunately many programs do not give average score of their class but if you are going to signal a program, you should search their name on previous couple of years spreadsheet and see which profiles are selected for interview.

A 260 on Step 2 means little if you lack real-world experiences that demonstrate your ability to connect with patients, work with a team, and advocate for those in need. Residency programs value candidates who have proven they can handle the responsibilities of being a physician through hands-on experience, not just test-taking ability. Strong letters of recommendation from mentors who have worked with you in clinical settings provide programs with a clearer picture of your work ethic, interpersonal skills, and potential as a resident. At the end of the day, programs want applicants who are more than just numbers. They are looking for residents who will contribute to their team, show resilience, and genuinely care about their patients. A well-rounded, humble, and experience-driven application speaks louder than any test score.

  • User:Have a WELL ROUNDED application. A solo 270 on step 2 doesnt matter if you dont have good LORs or adequate research. Your CV should promise dedication to your specialty of choice.

  • User:Plz plz plz set aside atleast 1 month to make your program list. You must remember that it is essentially 600 IM programs, 600 FM and idk how many else if you will dual apply. I could barely get through IM list and I wanted to dual apply to FM, hardly got through 20 Fam programs. Do not schedule USCE for September, you will be drained and you will have no time to decide properly. That was my biggest problem, I was not aware so honestly I did the best I could but it could have made a huge difference in my application. I had to choose my signals last minute also and I was sooo busy and so many issues at once, wallahi idk how I did it, by the Mercy of Allah SWT, but it would have been better to not be in that position. Lastly, decide your list before USCE so you can strategically do USCE in IMG friendly hospitals where you have a chance.

      • User:😭😭😭 dude on top of that I was having a housing issue, had to leave the accommodation suddenly and find a new place to stay, was ubering which was so expensive cuz this kind lady let me stay in her home but it was far. Alhamdulilah Alhamdulilah ya Rab for getting me through all of that 🤲 praying we all match 🤲

  • User:In my case, I felt visa requirement was a major factor. I applied (PM&R) exclusively seeking H-1B and NOT J-1 under any circumstances (already on H-1B for research and I just don't want to be subject to the J-1 waiver). Scores definitely helped, research and LORs helped a ton. Networking at conferences was also good. I would have liked to have more rotations/clinical exposure but due to life/work wasn't able to do more.

  • User:The idea that step 2 score only is what you need is dumb and Ill-advised.I have seen people match urology with 230 step 2 score.

  • User:Biggest impact was connections. Attendings, both at a Clinic based rotation and an observership got me interviews. I would highly recommend combining strong connections with gold signals.

  • User:I had a good application, weak on research. Visa requiring IMG. I believe the thing affected my app the most was to be a native spanish speaker. Other small things include having different hobbies, different work experiences not only in medicine. One advice, try to get USCE in hospitals you are going to (and can) apply (I did some USCE in a hospital which IM program didnt have any IMG, so bad luck with that. Got a good LoR though)

      • User:I should have specified, it affected positively my profile. A lot of programs told me that they actively look for Spanish speaking residents. I didn't have any issue with my accent, I was even commended once for my English

  • User:You have to be super smart with the signals. Check the programs in advance. Aligns your signals with geographic locations.

  • User:Letters of interest do work, and can get more IVs than you would expect. Utilize it if you think you need it.  Send before submitting your application and follow up a couple of weeks or months after submitting.

  • User:Take notes from the programs at the beginning once you start interviewing. The differences between the programs might be a blurred line when you'll need to do your RoL, otherwise. I am talking red flags, vibe and everything other things you might notice from the attendings and residents on interview day.

Get as many letters you can mint from as many specialties as possible, and plan step 3 ahead, you will save a ton. 4.   • User:1. Don't do clinic rotations. Preferably do a mix of rotations - big names + hospitals that have in-house residency programs where people similar to your background are present. 2. Be genuine and honest. Don't lie on your CV. 3. Start publishing and put your name out there. 4. Get your LORs uploaded early. 5. Get your PS reviewed multiple times. 6. Reach out to your potential contacts much earlier and stay in touch with them at multiple points. If you reach out to them only during your application submission, chances are they would be recommending some else instead. 7. Be prepared for your interviews and don't wait for you to receive the invite. Some of my friends who got just a few especially later into the season were waiting for the invite to start practicing but ended up having to schedule the interview in a day or two having no time to prepare/ create your story. Often times, we might have a great profile but may not be best at talking about ourselves. 8. Eat well and don't get sick, especially around your interviews. 9. Unpopular opinion, work at hospitals during your interviews- sharing snippets of your day is what the interviewers love to hear. 10. Develop a hobby and learn Spanish.

Research in your specialty of intrest marks a huge difference 6. Never loose hope - staying consistent and resilient is the key Common Mistakes to Avoid: • Applying too broadly or too narrowly—balance is key. • Not networking enough—connect with residents, attendings, and mentors. • Weak or generic personal statements that don’t add value to your application. • Delaying Step 2 CK or not taking it seriously. For minor but high-impact factors: Timely application submission, proper formatting of your CV, and good follow-up emails after interviews can all make a difference. However, if you were expecting networking, advocacy, or mentorship—such as introducing you to program directors, making calls on your behalf, or helping with research—then just an LoR might feel insufficient. Some supervisors are simply not in a position to provide extra support beyond writing a letter, while others may not want to overextend their involvement. If the LoR is strong and personalized, it is still a good contribution to your application. If it’s generic, it might not add much weight. If possible, try to build additional connections with faculty who are more involved in mentoring IMGs.

Hey everyone,

We’ve officially done everything we could for the Match, and now it’s just a waiting game. Just wanted to remind everyone to double-check that their NRMP status looks correct—this is our last chance to catch any issues before the results come in.

At this point, all that’s left to do is take a deep breath and try to relax (easier said than done, I know). Whether it’s binge-watching a show, hanging out with friends, or just sleeping off the stress, let’s take this time to recharge.

  As ROL deadline is approaching along with Match results I just want to wish everyone the best of luck!! We’ve been through way too much BS individually but I know we can all relate to how daunting STEP exams have been, mental health and this dreadful long process of residency IV’s. But at that moment all my struggles flashed before my eyes, all those uWorld subscriptions, searching for rotations, the constant anxiety about where to signal and all the paperwork done but how easily I just said I am applying for internal medicine and waiting for my result! It made me wonder How we all as a community have normalised the amount of hard work we do with no expectations for results.   My country will definitely be included in the new travel ban. Its insane that after spending years studying and thousands of dollars and (potentially matching) I will not be able to pursue my dream because of something as stupid as this. At this point I hope I dont match this way ill have myself to blame. This is so unfair I cant believe it. I wouldn’t wish this on my worst enemy.

I learned how to do things I had not previously done, I made a network that will support me regardless of the outcome. I wouldn’t go home with my tail between my legs… I pursued an opportunity and saw it through. Honestly the real question is how bad do you want it, if you truly love the specialty you say you would… would you go through training in your country (in the conditions that drove you out in the first place). If the answer is you’re not sure, then consider options that will allow you to live here outside of residency. I personally would return and do general surgery. And I know how low the blow of seeing your ex classmates graduate (literally a few days ago) as attendings or start subspecializing hits… but I regret nothing. :)

  I have read hundreds of reddit articles recently, and it is also the time when matchday is ready to come. The mood was excited and anxious. Those with low scores envy those with high scores, and those who have failed in step envy those with low scores. Some people are afraid to go to the project they don't like, and some people only want to have one project match. Some people are struggling to decide which interview to pass up because of the journey. Others just wish they could get an interview. Some can apply for elective, others can only be observers. As a medical nerd, I just want to say that you are all my envy. I look up to you as an example.

      • User:I think studying in Europe, my high step2 score of 269, high class percentile (98th), and overall research (2 textbook chapters, 1 poster at the APA, another publication) were the biggest objective things that helped separate me from others. My LORs, PS, interview skills, and my USCE (I did 3 months, but 1 month at Mayo) also helped a lot, but the things above probably helped actually get me the interview itself.

        • User:I also have a high class person tile but not from Europe. Do you think that actually matters as an IMG or was your University a well-known institution?

          • User:I think that where you do your medical school training does matter. Carribbean IMGs are sometimes looked down upon. I think if you're from an English-speaking nation known for having good training that can be some-what comparable to the US (ex. Australia, UK etc.), PD's know that it will be easier for you to enmesh into the US Healthcare System given shared cultural values and lack of language barrier. It's definitely an advantage, as all the IMGs (visa-requiring) from my class matched really well into specialties like Anaesthesiology/Radiology/Neurosurgery in USA/CAN.

      • User:When you do couples match, you make combinations where let’s say partner 1 matches into program A and partner 2 matches into program B. If each partner got 30 IVs, they can make about 900 combinations. I’m applying in the couples match this year, I got 5 IVs and my partner got 12 IVs. We have 78 ranks (since you can make combinations with no rank as well).

Hi everyone, 

The Good research project has recently gained popularity among IMGs in India for teaching research method and getting to publish a research paper. I did a paper with them and I highly advocate against them. 

Firstly, the charges are too high! Secondly, the quality of paper that comes out of their research paper is low. Our paper was rejected by multiple pub med indexed journal. They somehow push us to publish in Cureus. 

• The bottom line is that, it's just an utter waste of money and time. Please use it somewhere else to publish a good quality paper instead of being scammed by these people.

    • User:I have worked with many students for free, I never see accountability of even effort. I have spoken to Dr Sriya. She knows the limitations, It never made sense to me. I asked if the study design is already bad. How will you fix it with good writing? The papers I have worked on, I had infulencial mentors who grilled me for 6 to 8 months and some papers took 3 years to publish, that was just internal review. I think research cant be done in that small time frame, you might get a paper but cant do anything meaningful.

      • User:That’s correct. Students want everything for free. When TGRP started, it wasn’t charging anything. But students won’t do anything. A simple task like signing the contributor form would take 2 months. Taking fees is a way of accountability, so that only students who are genuinely interested can join us. Additionally, a research takes lots of efforts and mentors are involved at each and every stage, right from topic selection to article editing, modification, and publication and answering peer review queries. Mentors are doing a great job and investing their time and energy for this. That being said, individual mentorship is offered at TGRP, but students aren’t interested enough. Everyone just wants to get it done asap and fast. The expectations of students are to finish the research fast with minimal efforts and yet publish in a top-notch journal. And if they “publish” the article, they blame the mentors that they don’t publish in a high indexed journal. But the student is minimally interested in putting efforts.

And another one in Cureus. My letter to editor is now in a journal under peer review. Maybe the issue with your research group was different. But for me the entire research process was very systematic and I am satisfied with the research mentorship. I enjoyed the research learning process.

    • User:It’s neither good or bad, it’s an open source where I think all of the papers have to pay an APC. This reduces the quality of the research.

    • User:You can also name reviewers of your choice. (It means your friend/relative can also review your article) There are plenty of better journals than cureus

      • User:Even in top journals like JAMA, and several other journals you can mention names of the reviewers you’d like to invite to review the paper.

    • User:Cureus is not a bad journal. Everyone has different opinions. The general recommendation is PubMed indexed journals > Cureus (which is also PubMed indexed) > Scopus & Web of Science indexed journal > DOAJ indexed journal .

We genuinely try that the article is submitted in 2-4 PubMed indexed journals, if not accepted then submit in Cureus. There was never a “push”. My point of this post was to tell peers not to invest 20,000 rupees and then having a journal published in a non pubmed indexed journal. Or just cureus ( pubmed indexed). And since your team works on similar research topics I'm sure you'll know the pros and limitations of the paper, by now shouldn't you be suggesting journals where we might have a good chance of publication? What's the point of aiming at IF 4 when we already know we won't get published in it..

        • User:There are 1000s of journals out there, even PubMed indexed ones. Publication is about finding the right fit for your research article. And in several groups, even though the article was rejected by journals with IF 1, it was accepted by journal with higher IF. So it is all about finding a best fit and that’s what we try for.

      • User:It is glad to hear that you enjoyed the mentorship. But not that the fees is a mentorship fees. Your expectations were different and we never ever mentioned about publication. We never guarantee a publication at all. TGRP is not a publication service. TGRP is a research mentorship program that teaches students about research and helps them do research. The topics are based on a template. That doesn’t make it similar. Plus this is a teaching platform. And 70-80% of students who join us are beginners and for all these students, this is a valuable 1st experience in research. And there are 5-7 members of research team involved in it. Research mentorship and learning is different and publication is a different aspect. Kindly do not mix it.   This whole Usmle process has taken more than I have in me. I have been anxious about the Steps since 3rd year of med school. I have graduated for close to 1 year now and will be taking step 2 this month. I am studying for it. But I don't feel stressed or happy. I am just very indifferent. When should one know that this is it? I don't feel like doing anything. Just a body which is living till death comes. I will settle with perhaps less money but maybe sanity. When should one know that this chapter needs to be closed? Anyone else who has been in a similar position please help.

  • User:Sorry that you feel that way. What helped me was that I took this exam as just one aspect of my life. Never revolved my world around it. Stayed connected with friends, pursued my hobbies and continued living life. I considered this journey as something that i am trying to achieve and is a bonus if i actually pass the exams and match. I dissociated myself from outcomes. I always thought if this does not work out, well I tried and there’s better stuff out there for me. Take a break and ask yourself again what your purpose is. I hope that makes sense.

  • User:Sorry to know that you feel this way, but believe me it's quite common when you are struck at the same level it's frustrating to pass over that, in your case I believe you have been studying for step 2 for nearly a year now, which has taken a toll on you, take a break if that helps reorganize your schedule, don't dedicate yourself entirely just for studies dedicate your time for family and friends, working out and hobbies, this way when you are at studying you will be more dedicated and when at other stuff you will be at relax, hope this helps.

UK grad here,

The process seems sooooo incredibly arduous and long. I understand for someone from Indian or Pakistan it’s the pot of gold at the end of the rainbow but for me, I don’t know if it’s all worth it.

There are other issues in India (corruption, pollution, working hours,violence against physicians etc) that makes some physicians leave India. I would advise getting some, pay for it if needed. At least couple months, don’t do telerotations, if you have friends then just ask them.

  • User:Hot take, you just offended a bunch of people here and because of that everyone is discouraging you from this pathway. Nobody is actually telling you the truth. The process is arduous and it is long. Anyone here pretending that they have never had any thoughts like that and saying that because you had these thoughts that it is not for you is DELUDED. The burn out, the guilt, the fear, the embarrassment will be a part of this entire process with you until you have an attending job. And EVEN THEN, those feelings might be there If you want to do it, then you have to recognize all of those feelings and still push past it. The time will pass whether you stay in the UK or you go to the US. And it is not like these feelings will not be there if you stay in the UK. Getting onto a scheme is hard, takes time, you still have to run the rat race to build up your portfolio. If its your dream to be in the US, then why give up on it. You have to be delusional about your dream, that's the only thing that will make you be able to do hard shit (i.e. matching into the US). This is true for anything in life as well.

I personally think that the wealth that medicine offers you allows you to send your kids to private schools and avoid a lot of those concerns. Being in America is hard if you are poor, but life is easy if you are rich. Your kids will hopefully experience that rich life and I cannot imagine that you will damage your kids because of it. Also, to play Devil’s Advocate, if I were to make a generalisation about the use of recalls being limited to certain demographics then it wouldn’t even be a false or unfair characterisation, so you can’t really claim unfair bias in that situation. The hard work it takes to become a physician in any setting is irrelevant here. Also - anyone choosing to leave a country where they are already able to practice medicine, undergo a ton of difficult and expensive tests for the hope of practicing somewhere like the EU or US is being opportunistic. That isn’t a bad thing, people should take advantage of any opportunity they’re interested in if they feel it will provide them a better quality of life through professional advancement or financial compensation. Again, stop taking it personally. The data is the data.

Of course, when corrected for the Cost of living, this would be equivalent to a resident in the US. I will mention that in my state residents are paid more than other states Housing is often provided by the hospital/medical school.

  • User:I’m from a country where I found it necessary attempt this path but I was feeling like this before I took a leap. The anxiety and stress from even thinking of how tedious the process is enough to deter and mess with you mentally. It took so long before I even had the courage to attempt my first exam because I really feared the disappointment of things not working out. One thing I told myself was “one thing at a time” I took one exam and then from there another and then found myself applying and now just praying for the best. You have to just decide it’s what you want to do and go with the flow. Prioritize other things in your life and if it works out, it does. If it’s really stressing you to the point where it might be affecting your health, then it’s literally not the end of the world to not do it esp being from a good country. Also there are other countries like Australia.

  • User:Listen,as a person who has to travel to multiple cities to get their B1/B2 visa via an agency, they told me that it is better to have a reason other than rotations as the reason for going to the US. My reason was going for a medical conference,which I got an invitation for,and when I went to the visa official,she was asking me more about the medical conference and how I got the invitation as an Indian medical student etc. The main thing I tried to emphasize while talking to her was to say I was just gonna stay for the conference because I have no friends or family in the city(luckily they didn't ask me about family in the other cities which I did have) and that I have to come back and finish my internship in my college immediately after the conference so I can't stay long in the US. I had my 6 months banks statement and my dad's 6 months salary slip with all the relevant documents but they never asked me for anything just because I emphasized how I was not gonna stay in the US for more than 3 days due to my bigger concern being to come back and finish my internship.

    • User:This is a valid reason considering you are a student, but in my case with multiple rejections and all of them for observership and now trying again with a conference to attend will make them even more suspicious. They would have all my purpose of visit during my previous rejections recorded in the system. So will it be wise to try other reasons for the purpose of travel on B1?

      • User:If nothing else,use step 3 as a reason to get the visa. If they ask why,tell them that the exam can only be done in the US and can't be done in India,which is why you have to go there,but if they ask you what's your plans after step 3,tell them you're working in a hospital or something in India and you have to come back or they won't pay for your salary. If they ask why you applied and got rejected last two times,tell them that you wanted to get USCE early but didn't get it unfortunately so now you want to just write the step 3 and say your main focus is applying for India and if India doesn't work out, think about other non-US countries which would accept USMLE scores(New Zealand,UAE,etc).

        • User:I answered same for this The visa officer obviously didn’t care about that . Whether the exams could only be taken in the US or not which I clearly emphasized . I had step 3 letter plus my Scheduling permit . It still didn’t matter . US visa interviews are based on who you meet at a particular time . And once you have a visa denial , they’re biased towards you to begin with. In my experience, if all the questions you’re stating will be asked , you’ll get lucky to answer them , but they have no time asking you all that . Just 2 or 3 questions. You have to put all your answers in one , linking them to create a story as to why you’ll return to your home country . But again it’s based on luck. I’ll write my country’s board exams as a back up plan and USMLE plus residency as secondary one if I were you . Well that’s what I’m doing now . I’ve learnt in this journey not to put all my eggs in one basket otherwise I’ll get frustrated. And the most important thing is to live a life where I’ll be absolutely in control of my life not because some visa officer have to grant me a visa so I meet my life goals .

If you financially can invest, I would recommend you apply in the upcoming season. Play the cards you are dealt to the best of your ability. Network, send letters of interest, reach out to alumni from your uni who made it, reach out to PDs, engage in the open houses, and social events and insA youll find an opportunity. Good luck! :)

Travel history, if you have is definitely a plus point which shows that you are abiding by visa rules of other country and have always returned. But, during the interview, you should be able to say with conviction that your intentions are to return to your home country. For this you have to show your home ties, it could be family relationship, your employment contract letter or any other stuff (check us consular b1b2 visa videos on youtube for more details).

  • User:I would suggest to think about reasons why your B1 visa is getting rejected. I feel B1 has the best chances among all visa types because it's just a tourist visa. You will be going and spending your country money in the US. Whereas in other visa types, you will be earning there and bring some money back to your country. So think about why your visa is getting rejected.

    • User:They give the same paper to everyone, section 214b. I had all my home ties documents, answered their questions appropriately with confidence, still have no idea why am I being rejected.

      • User:Yes, I understand they don't tell the reason. But you have to think about it. Watch YouTube videos and talk to someone who has given multiple interviews. Remember each and every part of your interview, you will be able to figure it out. A lot of times, the way you speak matters too. A stuttering voice shows underconfidence and has been a reason for rejection, a lot of times. This is important because they won't care even while rejecting your J1 visa, even if you get selected in residency. They are brutal and they don't care about anything.

Never claim to have a relative in the USA for non-immigrant visa is a red flag. 2. You don’t need invitation to get B1/B2 visa. 3. When you get your visa you can do whatever you want in the USA as far as you’re not going there to earn an income 4. Some of you don’t get your story straight prior to your interview, therefore when they ask you questions you become nervous. 5. Practice your answers and watch YouTube beforehand, don’t just show up to interviews like that. 6. If you’re religious, pray to God for confidence and smooth visa process and have faith that you’ve already gotten the visa. NB: i have 5 years validity for B1/B2 visa.

And I know friends of friends who applied without usce get 2-3 ivs, not a guarantee to match or get ivs but again it’s a chance you might be willing to take. You can try J1 research visa as well if the B1 doesn’t work again with the changed reason- if you haven’t tried that yet. Good luck! :)

It is unlikely there will be a complete denial of visas, that would be extremely extreme and unlikely as residents are needed to keep the US healthcare system functioning.

I'm not sure but it says Jan 27th of that same year back then But the thing is, switching from a B1/B2 to J1 necessitates leaving the US and reentering, so I'm not sure how that was worked out back then. Just stay positive, we worked hard to get this far, InshaAllah there will be a way!

If SOAP is part of your backup plan, early preparation can make all the difference. Match Week is just around the corner, so now is the time to get everything in place.

Here’s what you should focus on:

– Certify and submit your MyERAS application if you have not done so already during the 2025 ERAS application regular season. 

– Update your Personal Statement and LORs. If you're applying to a new specialty, consider making your PS specialty-specific to demonstrate your strong interest and commitment to that field.

– Ensure a stable internet connection, a quiet interview space, and backup devices to stay prepared and respond smoothly without interruptions.

– The Unfilled Programs List becomes available to unmatched or partially matched applicants on March 17 at 10 AM EDT. Be ready to shortlist your top 45 program selections ASAP! (Reminder: Do not share this list with anyone as it is a Match violation.)

SOAP Fees:

Hey everyone,

I’m a non-US IMG feeling pretty down about not getting any interviews this cycle applying to IM. I wanted to ask about people’s experiences with SOAP—how it works, what my chances are, and if anyone has had a positive outcome.

My credentials:

Step 1: 207 (had issues during COVID)

Step 2: 250

4 LORs

1 year of research (no publications yet, but most of my work is on large clinical trials. I’ve gained great experience and can confidently run research independently. Expecting ~10 publications, mostly as 1st author, in my second year as a research fellow.)

~4 months of US clinical experience

2 years of graduation 

I know SOAP is competitive, but I’m hoping to hear from others who have been through it. Any advice, insights, or success stories? Would really appreciate any guidance.

  • User:With a 250 on Step 2 and no attempts, it’s very surprising that you didn’t get any interviews. It actually makes me suspect your letters were not good, at least 1-2 of them. Or…if you wrote a “strange” or ineffective PS, that could have hurt you. Obviously there’s not much time left, but if it’s at all possible…get new LoRs, perhaps just for any letter you suspect might not be good.     • User:Thank you! I am sure I have at least 2 outstanding letters as they are from people I worked closely with and they have been very supportive. The other two are average or even below average. My personal statement reviewed by more than physician involved in fellowship interviews. The only mistake I did was not mentioning the step 1 thing at all! Because I thought since i am competing with pass/fail anyway it shouldn’t be an issue. I think that’s where I went wrong.       • User:I don't think your step 1 was an issue. my score was really low, yet nobody asked about it during my IVs.

  • User:You were filtered out. Were you able to use your connections? Other than the Step 1, you dont have any red flags

    • User:I used my connections as much as I can and got a feedback in indirect way that my step 1 score is the reason. My mentor suggested that I rewrite my personal statement and highlight the struggles I had during step 1 and that’s what I plan to do next cycle.

      • User:Don’t do that, that’s what the impactful experience section in ERAS is for ☺️. The PS should be positive.

        • User:If i update it now will it show on soap? I really had no idea what to write in impactful experience. I didn’t want to make it that much of deal but i have to now.

          • User:No, unfortunately you can’t add it now for SOAP because you can’t change your CV at this point. And, honestly, if you were getting filtered for your Step 1 score…those programs aren’t reading your CV or PS anyway. Personally, I don’t think not writing about Step 1 was the issue. I’m happy to review your CV/PS and give you my input on whether either of those documents might have affected your interview season. I see 100+ applications every year, for the last 11 cycles, so I have some foundation from which to offer this feedback. DM if you’d like ☺️

I applied to 85 programs, which wasn’t ideal, but money was a factor. Next year, I’ll apply more broadly. I skipped the most competitive programs and focused on ones with good research opportunities since I’m really passionate about research and want to keep it part of my career. I ended up applying to a mix of university and high-tier community programs.

  • User:How broadly did you apply? I know signals have kind of changed the game, but still always apply broadly because you never know if you get an interview from a non signaled program. SOAP is kind of unpredictable and honestly does not favor IMGs, but I still know of people that have matched during SOAP. Maybe it works out, but also see how you can improve your app for next year. A good way is applying with step 3 passed

      • User:You have good stats. Don’t be discouraged, it’s rough. Recs in case you don’t get a spot in SOAP and apply next year: get step 3 out of the way, review your personal statement and CV. Have multiple people review them, even people that are not necessarily in the medical field, but in the corporate field that do hiring (in case you have friends that work in those fields). Have Americans review your app, not just other IMGs, they provide good insight on what is important. And apply as broadly as you can. I would say to not feel discourage and maybe try to apply again to university and high tier community programs, but try to include lower tier as something to fall back on to. Also, i know this may be more difficult, but try to see if the LoRs you had were good, and if not try to get new ones. And network network network! You got this!

  • User:Number 1 rule for SOAP: Apply within minutes when the SOAP starts. I had my own list ready on another computer with my partner ready to search. The moment the list appeared, I searched the program in my list and I had comments written (IMG friendly, not friendly, sponsors visa, doesn't sponsors visa, accepts attempts, no attempts bla bla). If you fit the criteria, APPLY. I got 1 interview as non-us IMG the first day. The interview went well, I was not given a spot because they had issues sponsoring visas(they updated their website later). So ACT fast, that's your only chance to get their attention. This is what I think!

    • User:This is not true. Programs cannot view applications until Tuesday morning. You have all day Monday to research programs and apply.

  • User:Hey! Since you scored good on Step 2, the step 1 score isn’t that big of a deal. I believe you should get an LoR for FM, EM and apply to these specialities as well in soap. AND PLEASE PRACTICE FOR INTERVIEWS DAILY UNTIL SOAP! You already improved up and beyond your step 1 score, it is just a way/excuse for the connections to find their way out of not helping you. Your PS should be about you as a person, and what makes you extraordinary, not your shortcomings. And I personally don’t think you should mention a lot of Step 1 problems even during interviews (until you’re asked), just focus on telling them how you improved during Step 2. No one wants to hear the sad story, just the one where you triumphed. Good luck!

Who else is going through this phase where you feel like you're upto no good?

 Last 5-6 months I've been doing research of programs to apply to, then waiting anxiously for the IVs and then giving few interviews eventually. It seems like an eternity being a real doctor and I'm seriously doubting my skills and knowledge as a physician.I feel like I'm upto no good and the thoughts of possibly joining residency (apparently the toughest time of a doctor's life) is haunting me.

I know this post sounds very clingy and open. But it comes from a place of anxiety and stress

Despite scoring good in step 2 ck, having 3 months usce and 0 YOG. I ended up with 2 IVs. Not here to rant about this, what happened, happened. 

• The interviews went great from my end, vibed quite well with the interviewers. But I'm just anxious of not being matched. Any idea how much of a luck do I really need 🥲

I got my visa rejected in my 1st attempt I had 2 invite letter 1 from hospital and 2nd from clinic letterhead but I think he didn’t like the idea of clinic one so he rejected my visa. Now I am super confused of what to do next:

 Should I reapply again immediately?

Should I reapply with the same hospital letterhead as it was the one he liked first before I showed him the clinic one ?

Or should I get another hospital invite letter and go with that ? But won’t that be suspicious?

  • User:Which country are you applying from? In any case if you have an invitation letter from a University, it will not be rejected. Many clinic rotations are blacklisted by the US embassy as well as PDs. DM to ask about India.

          • User:Oh, I am sorry to hear that. In any OFC, I think a clinic letterhead is inferior. Try booking in Delhi/Hyderabad next time ahead. Use your hospital based letter. Make sure you tell them that you got through cold mailing faculty or through a connection. Do not mention any agency. Polish your English language and make sure you are well dressed, clean shaven and very confident. I'm sure it will get approved the second time.

( No official stats exist to justify this, it is all based on general consensus)

Recently Trump has banned people flying in from Afghanistan and Pakistan. It shouldn't affect skilled professionals such as doctors, but students going for rotations on B1/B2 will find it harder to secure visa for sure.

                • User:Not really. For hands-on experience only students are allowed. If you're a graduate securing a visa will be harder because as a student, you're guaranteed to return back to complete your course and for graduation.

  Im just curious if its a computerized algorithm and everything is set on march 5, what happens during those 12 days.

    • User:Honestly I think it’s probably a throwback from when match results had to be mailed out (when the match was created IMGs weren’t allowed in the match and there were a ton more spots that medical students and emails weren’t common) and when emails came along no one wanted to change the system.( also explains why urology who uses their own early match but also NRMP just changed it to finding out on the day)  TLDR it’s likely a holdover from when email and instant communication wasn’t a thing

As a US grad honestly feel like they could bring it down a week but it is what it is

Ik this is rough. Hypothetically. 1 interview. 21 spot at program. 200 interviews. Let’s say I ranked in spot 100-150. What are my chances. What are my chances if I ranked 200. And what are my chances if I ranked 75. My mind is thinking of all scenarios. Just need some odds.

The average im program requires to rank 7 to fill one position meaning that on average the last person filling a seat in your program will be rank 147. As long as you are ranked below that you have a good chance of matching. Keep in mind all of these are averages and estimates and nothing is really concrete. However i do find it weird that program with 21 positions only interviewed 200 are you sure about that?

    • User:Had a program with 29 positions interview only ~220. Prob a good program so don’t need to go that far down deep to fill.

  I feel like my interviews weren’t amazing like some conversations with one faculty would be great but then with the other person it would be whatever. Not like everyone who interviewed at the program i had an amazing interview with them. So do people match with  mediocre/whatever interviews?

Hello everyone!

I am a **visa-requiring IMG**, currently doing clinical rotations in US, will be able to secure LORs in both FM and IM. Here is my profile:

1. **YOG= 2019 , Step 2=252 , Step 3=scheduled**

2. One year hands-on experience in home country, one publication

3. 2026-27 match applicant

4. planning to apply to **both FM and IM.**

5. future plan is to do fellowship in women health.

Those with similar profile, can you please give advice about:

How many IM and FM programs should I apply to?

Is **research** important for community programs?

• Do FM programs also demand hospital LORs?

  • User:YOG of 6+ years is tough, plus you need Visa so apply broadly and put focus on FM. 200 FM, and 100 IM to community programs and less competitive regions/hospitals.

    • User:Totally agree with this. Signals may ultimately change things up in this regard, but I saw too many people not get IVs from signals, yet get interviews from totally unexpected and off-their-radar programs. If the goal is to get as many IVs as possible, applying to less than 100 doesn't make sense. Anything more than that is great, depending on financial feasibility of course (and one shouldn't apply to programs where they don't meet minimum requirements to match or to programs with no IMGs).

    • User:Thank you for the advice! Yeah, my plan is also to focus on community programs. Can you please share your thoughts on research versus USCE significance, esp for community programs?

      • User:usce more important. Get a couple case reports or abstracts if you can.

  • User:You will need step 3 as a minimum , FM will be tricky because FM has a hard on for non visa seeking applicants and YOG <2 , but you can get it if you outreach programs IM will like you, but your YOG will be a turn off for them too I'd suggest get as much USCE you can and step 3 and split IM and FM equally , 100 -100

  • User:I don't know about FM, but for IM you should apply broadly (i.e. 100+ programs at least), this is not to say that you have bad stats, its just that things are much harder for us non-US IMGs, apply for as many programs as you can, the one main determinant factor in getting 1 vs. 10 interviews nowadays is your visa status. Your stats are great, I've seen people with IVs with lower step 2 scores, and people with no IVs with higher step 2 scores, you need to take care of your CV and PS, and one more thing is that clinical experience (yes even in the home country) seems to be prioritized in some programs. Take your time, keep your chin up, and you will do great inshaAllah brother!

      • User:Personally I think USCE and US LORs are far more important, but get a second and even a third opinion, step 3 opens the door for an H1B visa too so that’s something to consider

        • User:Also do not get depressed over the YOG thing, I know a doctor who matched after 10 years, granted it was through a connection but still. I’ve seen people who match with longer YOG, less step 2 score and overall less impressive application. What Allah has planned for you will reach you regardless, just stay on your path brother and inshaAllah you’ll reach your goal!

  • User:Step3 is very important for old grads. Need it before submitting eras

  • User:So you're going to get a lot of people saying to apply broadly. That is the classical advice for IMGs but I disagree. \- Signals have become a big thing since COVID. Most programs are using filters to remove applicants who didn't signal. If a program gets 3,000 applications and only interviews 300 people, signals will determine who even makes the initial cut. \- By over-applying you're wasting time and money applying to programs who flat-out won't consider you. If the (fictional) North Montana Nuclear Medicine Program consists of 100% AMGs, you as an IMG are most likely not going to get an interview there. My recommendation is to apply to **30** programs per specialty. This saves time and money, and is also the point at which ERAS fees start to climb sharply. This also gives you a solid goal to reach with respect to researching programs - you can go in-depth and determine if they're truly IMG-friendly, if they're only accepting Caribbean IMGs, or if they're IMG-hostile.

  • User:If you do want to do a fellowship in women's health then you should ideally stick to applying FM and not try to split between FM and IM, which sould end up hurting your chances of matching, and moreover that you're going to encounter way less womens health in IM residency than you would in FM, so I feel like it would be smarter to just apply FM. I'm saying this from my experience in FM rotations and what the residents there told me.

  I was dismissed from residency just close to the end of my intern year. I reapplied this cycle and got a couple of IVs, I am just so anxious and just want to see if there are any success stories of matching after a dismissal and how they improved their chances.

I realize your case is likely more complex, but even when you're 100% right there are generally lessons to learn. If you end up in SOAP, just lean into lessons learned and demonstrate you're teachable in your PS. You can say that due to legal reasons you can't discuss fully, but this is what you learned from the situation, and this is how you will avoid ending up in a similar situation.

• I am from Pakistan. I have been accepted as a paid postdoctoral research fellow at the Duke University. They said they will be offering me J1 scholar visa. I am worried whether recent travel ban will affect my visa approval in anyway or will it be primarily on b1/b2 holders? I know no one knows for sure but when Trump administration decided to put ban last time. Was it on all visa holders and if that was the case, were medical professionals and research fellows affected?

I sent a LOI to a program that I was sure would be my #1. Circumstances have changed and now it is more important for me to be close to my wife who is entering masters degree in a city where I had another interview. I don't know what I should do. I know that many programs do not care about LOIs and in fact I found out that the program to which I sent my LOI sent RTM and Highly ranked e-mails (I am not one of those to whom they sent).

  I am a non-American doctor who took the Step 1 exam for the second time and passed. I also took the Step 2 exam and scored 265. I faced significant challenges, which is why I was unable to pass Step 1 on my first attempt. However, I have now successfully passed it. Does this reduce my chances of securing a medical residency, or has obtaining a residency become impossible? Can you help me?

  • User:It does “hurt” your chances to some extent, because there are programs that have hard filters for attempts on the Steps. But with a 265 on Step 2, you have a strong chance of getting interviews ☺️

  • User:At the end of the day it is upto you financially. I feel like if you networked well you have a chance of matching and you can explain your reason very well. Good luck!

      • User:Yes, you'll be fine. Step 3 will be critical. USCE in the specialty. Get support on your ERAS app and PS. Research programs carefully.

          • User:Yes. It’s a sub specialty so that’s fine.

  • User:Apply broadly and focus on programs that dont filter based on failed attempts

  • User:Tbh many programs will likely filter out your application based on Step 1 fail, however your great step 2 score would likely still garner you some invites. Really depends on which specialty you want to apply too. Many have matched with failed attempts, but it is not easy, especially for a visa requiring IMG. You should focus on completing Step 3 with a high score and getting publications in the field you want to match.

      • User:With a strong personal statement and US LORs I think you would get interviews. You might want to dual apply family medicine if you want the best chances to match.

Going for rotations in the US in a month and I can’t begin to describe how anxious I am!

Everything including the visa, selecting a rotation, booking flights, looking for accommodation and especially the fear that I’m gonna leave my parents and go such far away makes my head spin.

This constant fear that something is gonna go wrong and everything will fall apart. I also think that maybe I’m not made for this journey considering how mentally tiresome it can be.

It’s so scary being an adult. Figuring out life. And the uncertainty of this USMLE journey makes things so much worse!!! 

• Please Tell me it will get better. Can people who’ve been through this journey offer me some insights on how can I make this easier for myself coz tbh it’s affecting my functioning as a human being.

  • User:I am currently doing clinical rotations in US. As a mother of a toddler, going for USCE was very stressful. The first week after landing here was very rough. I had severe jet lag, My credit card was not working, I missed my domestic flight consequently have to wait for additional 3 hours at the airport. But one good thing was that I have some friends/relatives here ,who are in other states and they helped me order online grocery. My eSIM was not fully activated because it was my first time using eSIM and I didn't know much about it, but it's been 10 days here and I have adjusted pretty well. In short, it will be an adventure so hang it tight and you will overcome it. In US, every system is structured, once you understand it, things will be simplified. Wishing you best of luck!

Foreign land, will I be okay, will my rotation be okay, will I get an LOR, will my accomodation work, will I get homesick, how do I do xyz and so on. Piece of advice is to pause and embrace the unknown and the thrill of solving things and learning along the way. Don’t take it as a task but take it as a once in a lifetime experience. Be excited about the possibilities that can happen in this journey. I came back with meaningful friendships, lasting relationships, a lot of life lessons, learning about a city and knowing that this wont be foreign to me the next time i come here is a satisfying feeling. Hello guys, can anyone tell me about the australian pathway and how it compares the to the UK/US and if its worth it or not (easy to get a job?)

  • User:Be careful with the Australian pathway, the culture, its laws, etc. It’s much easier than the one in Canada or the USA, but it has its own complexities. The most reliable sources are their official websites, though they sometimes use confusing language. In general, passing the AMC is key. First, check all the available pathways and see which one suits you best. By the way, as far as I remember, working in rural areas is now mandatory for three years, meaning you must work in a rural setting to get your license at least for that period. Confirm this, but that’s my understanding. Another aspect many forget: Australia is the land of dinosaurs. Be careful where you go. There are places where people from Europe or Latin America aren’t used to not just the heat but also the wildlife and other environmental factors. Also, Australian laws are very strict. You must follow everything systematically, step by step. Don’t skip steps, or you’ll have problems later. You must go from step 1 to the last one no jumping from step 2 to step 5. This is not the USA. Other than that, it’s a great country with great people. Salaries might be a bit lower, but the quality of life is much better. If you like adventure, you’ll have a wife in three months. Good luck!

Worth doing the USMLE steps and trying for an anaesthesia or radiology residency? 

Zero publications. Zero research. Zero audits. Zero USCE. No steps.

Haven’t even done the steps (though planning to go absolutely insane with the studying and doing them within the next few months). 

I am thinking of flying over to the states in the next few months and doing a rotation for USCE and a LOR or two. Looks like I will have to pay a few thousand to AMOpp….dont want to say their name as I don’t do free marketing but I don’t think I have the time to wait for potential replies from cold call emails.

Reason behind all this? I can’t stomach the NHS anymore and with the disgustingly absurdly low salaries, I simply cannot provide the kind of life I want for the family.

      • User:I feel you , it's your choice , but know this that the USMLE requires a boat load of money (in the ball park of 20k USD), and requires a minimum of 2-3 years of prep time so most likely you would have to quit your job at some point , in your case you would require all the steps done (1-3) , minimum of 6 months USCE to even stand a chance , and even then if you require a visa your chances are pretty slim And forget Rads and anesthesia, you would be aiming for Peds and FM

        • User:20k? Sorry the USMLE STEP 1 is a little over 1k. Resources are free on the net. step 2 is the same. By my calculation that’s circa 2.5k. Where on earth did you get 20k from😂? A placement, for USCE and LOR, can be secured for a couple of more k. So we are already at less than 5k.

With radiology maybe try the alternative pathway, i know a few people (old grads, attendings in their home country) who started in radiology in the US that way. Anesthesia will be a bit harder. But you can eventually consider (if you score high on the steps) peds, pathology, IM and FM. The whole process costs a lot of money. You said in the comments that the steps were only 2k, but getting USCE if you don’t have anyone who can house you there will cost you multiple $$. Even more if you bring your kids with you.

Do a UK residency in the meantime, and once you become eligible, apply for a Practice Ready Assessment pathway in one of the provinces. It's definitely a time intensive process, but if you do your research and plan your residency/paperwork etc. accordingly, you can probably live and work in Canada within the next 5 years or so. Talk to an immigration lawyer for any "FastTrack" options for healthcare professionals. Or go to 'Straiya. It's always so nice out there, only everything wants to kill you. First, I have no idea where this entire thing of "Western" graduates being preferred over other graduates even comes from. There is no data to support this notion. By comparison, 34 Brazilians matched, 18 Brazilians failed to match. I've never seen a single Brazilian come on here and tell others that they will match just because they're Brazilian. The only country that has a real advantage in this is Canada. This idea that people will welcome you with open arms just because you're British is wrong. Second, you graduated five years ago (which in and of itself will filter you out for a lot of programs) but you're only an F1, have no research and haven't taken the steps. Your interviewers will absolutely ask you what you've been doing since graduating. Third, this entire process is really expensive. I'd guess at least 10k USD. Step 1 and Step 2 CK alone will cost you around 2.5k USD. You mentioned you'll pay "a few thousand" for an agency for USCE. I doubt the agency will cover your plane ticket, accomodation, food and transportation. Then there are the fees for actually applying to residency programs. The first 30 programs will cost you 11 USD each, and every following program will cost you 30 USD each. If you're applying to 100 radiology programs, that will cost you 2430 USD. If you apply to 150 rads programs, you're looking at 3930 USD. I don't mean to be a Debbie Downer. I think it's admirable that you as a parent want a better life for your children. I do think it's possible for you to match (though I honestly don't know how viable specialties like anaesthesia or radiology are). I believe it's worth trying and giving it a shot. I can't imagine anything worse than wondering the rest of my life "what if". But that's me. You have to figure out for yourself and your family if it makes sense to risk losing 10k USD, maybe even more. Best of luck to you

  • User:Do two year of pure research in these two. Belt out some first author papers in high impact factors(dont fall for meta analysis shit), it's possible but very uphill. Depends how much kid responsibilities you have. Minimal is ok. Even if it is around 70 30 between you and your partner, it's difficult yet doable.

  • User:Are you a practicing doctor in the UK? FWIW I didn't find the "YOG thing" to be true if you worked as a dr back home. Are you a UK anesthesiologist? If so, you can work directly; you can skip residency.

          • User:It's more complicated. You get a full licence after a few years in certain states. Depending on your specialty. In practice, it will be always harder for you to find a job and you'll always be reminded that you didn't train in the us (I'm repeating what I've been told). It's a personal decision and it depends on what you want and what you can as these jobs are harder to get than finding a residency.

  • User:Did you not complete FY2? Are you not in training atm?

  • User:Just move to the gulf and you’ll make close to what US physicians make post tax

  • User:Wouldn't you make more money just working in a small city with a lower price of life?

  • User:Well, I'm not an old graduate, but I am old. I started the process at 30 and finished at 32. Very, very good score (245+). Connections it's that simple. If you don't have them, it's difficult. Have you tried pursuing another specialty? And from there, moving towards something similar, like pain management? By the way, some states are offering a 2- to 3-year supervision pathway if you have completed the Steps, followed by a state license. You should look for more information on that there's something like that in Florida. I know a Cuban who became a surgeon at 50, and another who, at 42, has already been accepted into a specialty, and they are visa applicants. So, it's definitely possible.P.S.: The more personalized your letters of recommendation from rotations and the more friends you have here, the better your chances. If you're going for IM, it will be much easier. Anesthesiology and radiology are very competitive for non-USMDs.

So I’m asking this because it’s been so quiet since interviews for me in the sense that, there are people I knew in the past that matched but they kind of always had a slight clue that they’ll match there either through their connections, or PD on interview day letting them know they really want them there, responses to letters of intent etc. 

• Of course they haven’t been told directly but kind of get the sign. Is there anyone that has matched in the past that just got the surprise on that day without any kind of noise during the wait period ?

SOAP interviews are a whole different experience from the usual residency interviews. They’re fast, unpredictable, and often informal, so you need to stay sharp and prepared for anything. 

Most interviews happen over phone or video, and they tend to be short and to the point. Don’t expect a long, structured conversation. Be ready to introduce yourself, highlight your strengths, and explain why you're a good fit in a matter of minutes.

The basics: 

* SOAP interviews happen between Tuesday (March 18) and Wednesday (March 19). Stay on your phone and email so you can respond immediately.

* If you’re outside the US or Canada, get a US number to avoid missing calls. You can use apps/services like Skype or Google Voice. Be sure to answer unknown numbers - it could be a program trying to reach you. 

* Clear your schedule for the whole week if possible. You’ll need to be available and focused. 

* For video IVs, dress professionally - suit or blazer in navy, black, or gray.

* Choose a quiet, well-lit space with a neutral background. 

* Keep your phone charged and have backups in case of interruptions or technical issues. 

* Keep your ERAS app and PS handy for reference.

I'm visa requiring Indian Non-US IMG done medschool in china. step1 pass in first attempt. step 2 ck - 225 . YOG jun 2023. no publication and US clinical experience YET, no connection or relative so far. Planning to do 4 5 month of clinical rotation and will sit for usmle step3 if i stick to this path. I wanted to match in IM but now i am flexible with FM and Pediatrics.

• i am going to married in end of this year. so Please give me BRUTALLY HONEST opinion. Should i countinue this journey or leave it as it will take many years to get matched now?? anyone who had same credential as mine and get matched earlier? how do you increase chance of getting matched early? will getting High score in step3 will compensate for my low step2 score to get more interview in next match cycle?

I dually applied to FM and IM, had step1 pass, step2 230, step3 237, 5 months of hands on USCE, research and volunteer experience and No Gaps, still didn't get any interview and now waiting and hoping for SOAP. While a friend had step1 pass, step2 225 and 3 months USCE, applied only to peads 45 programs without research or volunteer experience and still received 5 interviews. Apply to peads but align your whole application to peads

    • User:Ohh.. thats very informative… thank you very much … i checked last year match data .. in data , peds have highest match ratio in 221 to 230 marks range+ good matching probability all over…. But peds have very few programs to apply and didn’t hear from my circle taking peds ..so was unsure about that option

      • User:Yes, peads has very few programs that sponsor visas. Take it into consideration that peads also have comparatively fewer applicants. Peads programs don't care about your scores or attempts, so if you are dedicated to peads and your application screams "I love peads and take me in", the programs will take you in and you'll match.

        • User:Do you then an old graduate with 7 YOG will have a good chance in peds as well?

          • User:Yes, if you have all the experience in peads and a good personal statement, you have really good chance

            • User:My experience is a general physician I didn’t complete my residency yet

              • User:Do one or two rotations in peads and have volunteering experience with some sort of child welfare society or NGO. Add those to your experience section on ERAS. You definitely see pediatric patients in your general practice as well, so just tie your experience as GP with sort of a rewarding experience while working with kids specifically in your PS and you'll be good to go.

  • User:I have a similar background to yours although I’m a U.S. IMG now per their definition. I think you are within the realm of YOG needed, all you need to do now is network and get USCE. The most pressing question in your case would be which specialty you want to focus on. You’ll need to tailor your resume and experiences to it to stand a higher chance of matching next season.

I remember reading somewhere that it is 7. But I cant find it anywhere so i am wondering if i am mistaken. 

And if it was 7. Does that mean that for a program who has for example 10 positions. Anyone in the top 70 would ideally match if they ranked it ?

  • User:IM APD here. It depends on the program. Top 30 its a ratio of ~3-4:1 ranks:match. Mid-tier University 5-7:1. Community 7-15:1. Obviously, there's lot of generalization in there.

    • User:Interesting. Do APD/PDs share these results?

      • User:Not publicly, but based on my experience at different programs and colleagues' experience, those numbers roughly hold true. Varies a lot year-to-year, too. In my community program, if I've got a couple of local people who say have a spouse employed locally and they don't want to move, or they've got kids in school or something else that really makes it obvious they want to stay then that cuts my now long my ROL needs to be by 10-15 each. But I might not have that every year.

    • User:I received an email that i got ranked highly by a community program. Using these numbers does that mean that its guaranteed I match this year in their program (given i dont rank in programs i ranked higher)

      • User:Yeah, don't take anything as a guarantee.

  • User:Depends on how competitive the program is, obviously.

  • User:Good question!

  • User:Yeah it's an average. it's also why even by having one interview people can match; if it's community, rank can go all the way to the lower bottom.

I'm a final year med student who recently passed Step 1 currently applying for electives in the US. The thing is, I am kind of losing hope in getting an elective position. Is it possible to match ObGyn only with observerships?

For perspective: I currently have 3 published papers in good, Pubmed indexed Gyn journals (1 as first author), might be able to publish a few more in the near future, and also a couple of posters and oral presentations at local conferences.

• What should I do to have a better shot at matching ObGyn (aside from getting a good Step 2 CK score) and what would be a decent YOG for me to apply - taking in consideration I will have to work in my country for some time to raise funds before going to the US?

  • User:Try to get those USCEs, youll need those LoRs. Besides that, a good ck score and connections will get you the interviews. Yes obg is a surgical specialty technically, but its not as competitive as GS. It is competitive, but not impossible

    • User:As far as YOG, the less time the better. If you start going over 3-5 years, youre gonna start getting filtered out

      • User:What about doing home country residency for 4 years and then applying the next year

        • User:Can help, but the YOG will still hurt you. If you do the 4 years and then apply the year after, a lot of programs will filter you out based on a YOG >5

  Can anyone who has tried the agency or knows someone personally who tried them confirm that the rotations they promise exist and it’s not just a money scam 🫣

  • User:Highly overpriced. I believe it’s a one man company. Unsolicited advice but, exhaust all the options you have before you even think of going ahead with the agencies.

    • User:Thanks for the feedback! Very difficult to get inpatient rotations without agencies, most doctors I contact only allow you to be with them in the clinic. VSLO options are insanely competitive 🤦🏻‍♀️

  • User:Totally legit in my exp. Booked a couple of rotations last year,decided to drop the plan last minute and he was kind enough to refund most of it. I sold the remaining credits to an interested party i found on one of the IMG groups .

  • User:I did an outpatient IM rotation with them, had a good experience with both the preceptor and the agency :) they were actually very helpful in the process

  • User:I booked a rotation with them last year but I had to change it to a telerotation for some reason. They refunded a portion of the money for me with no issues, as the telerotation was cheaper than the inperson rotation. I didn't have any issues with the telerotation as well.

  • User:I did two rotations with them. One was good another one, they asked me to change just 2 days before. I accepted cos I had no choice. Preceptor works at two different hospitals. Then when I went to the rotation, I was told that I cant rotate at one of the hospitals. However I did rotate at the other hospital. Preceptor was cool, but rotation plus isn’t that transparent upfront. However he was in contact with me via whatsapp most of the time.

  • User:It should be ☺️ You’re right in the ballpark for average contiguous ranks for matched neuro applicants from 2024 (see [page 161 here](https://www.nrmp.org/wp-content/uploads/2022/07/Charting-Outcomes-IMG-2022_Final.pdf)).

  • User:Nobody can tell you for sure. Just look at NRMP data and see how many people matched who had 8 Interviews.
